How to refer to Deer Valley Hospice Care
If you believe that your loved one may need hospice care please do the following:
- Call Deer Valley Hospice Care at (314) 396-2211 during office hours of 8:30 to 4:30. After 4:30, please call (844) 337-3177 to discuss why you feel your loved one may need hospice care. Patient referrals are received 24/7.
- We will gather information about your loved one, including physician information and a brief description of your loved ones health.
- We will contact the patient's primary care physician or specialist and request a hospice order to evaluate and admit if appropriate. If possible, please share your concern with the physician prior to us notifying him / her. Let us know if the patient does not have a primary care physician.
- We will reach out to the patient, caregiver, POA or next of kin to arrange a consultation to discuss the hospice benefit.
The consultation can take place at any location convenient for the family, even at the hospital. - We can arrange the hospice evaluation immediately following the consultation or when it's convenient for the family.
- If a patient is in the hospital, we will work with the discharge planner to schedule the evaluation upon discharge to the patients place of residence.
