What Does the Hospice Benefit Entitle You To?
The Hospice benefit includes the Interdisciplinary Team. The team at Deer Valley Hospice Care includes a diverse range of hospice care professionals who meet every 2 weeks, or as needed, to discuss and develop a plan of care that meets the evolving needs and goals expressed by the patient and family.

Medical Director
Our medical director is responsible for creating a comprehensive care plan and ensuring that it is carried out by the hospice team and caregivers. They also make sure the patient’s family are always informed and updated.
RN case manager
Our RN case manager educates families and caregivers of the patient on recognizing potential symptoms and providing safe and competent care for the patient. They offer emotional and practical support for both the patient and their family or caregivers.
LPN
Our hospice LPNs provide constant and ongoing education and care for our terminally ill patients. They evaluate patient needs, provide ongoing hospice education, help create care plans, and provide end-of-life support to patients and their families.
Spiritual care coordinator
A major part of hospice care is tending to not just the body, but the emotional and spiritual needs of everyone involved. This may include arranging counseling for patients and grieving families, offering spiritual support to caregivers and helping with funeral planning.
Bereavement coordinator
Dealing with grief can be difficult. Our bereavement coordinator help the families ease the pain. From helping families deal with the diagnosis process to the actual loss, we do all we can to help our patient's families cope with these hard times.
Social worker
Our social workers advocate for each patients end of life goals and wishes and help individuals address the emotional aspects of late-stage illnesses. Hospice social workers also assist families in identifying other available local services and resources for additional support.
Volunteers
Our hospice volunteers do so much for Deer Valley Hospice Care. They work directly with patients, caregivers and their families to prepare meals, help with chores or even to just talk and provide companionship as needed.
Hospice aides
Our hospice aides are the eyes, ears and hands of the hospice care team because they have significant personal contact with patients. They also document and report physical and emotional needs to the team. The hospice aide will assist with ADLs and light chores.
Other Hospice Benefits
All medications, medical equipment and supplies that relate to the pain management and palliation of the patients terminal illness will be covered.
Non-Core Services
Deer Valley Hospice Care also offers non-core services like music therapy. Music therapy has been proven to help provide emotional support, pain relief, comfort and helps ease fear and anxiety.
